The goal of a Pest Management Alliance Project is to demonstrate and encourage widespread adoption of economically sound reduced-risk pest management. An Alliance promotes a concept of voluntary cooperative problem solving, which creates a climate where growers and urban and suburban residents are better informed and more willing to implement practices that work. It is important to identify project successes and accomplishments. The Project Summary form is designed to record measures of success for inclusion in the project final report.
Free reader/filler downloads and instructions on configuring your browser are available from Adobe (PDF) and CAERE (FML). The Omniform format allows you to fill out the form online with the free download. You can download and save the Omniform versions to your local system. You can also fill the form out and save the data to your local system. This means you can make changes to the existing data, as well as update (add to) the form with new data.
(12/27/2001--The Omniform Internet Filler is no longer available free of charge; but if you have already downloaded the filler, you can still use the fml version of the form.)
