TO: Registrants of Pesticide Products SUBJECT: APPLICATION FOR PESTICIDE REGISTRATION, REVISED (DPR Form 39-030) The Department of Pesticide Regulation (DPR) has revised the Application for Pesticide Registration Form 39-030. These revisions will make the application form easier to read and use. Enclosed is a copy of the revised form. It is to be used when applying for pesticide product registration or when amending the formulation of an existing product. Please begin using the revised form (revision date March 1999) in place of older versions of the form. The application form can also be downloaded from DPR's webpage at www.cdpr.ca.gov under Programs and Services, Registration Branch.
