Update to the Pesticide Episode Notification Record (DPR-ENF-114) for Reportable Incidents

Attached is an update to the Pesticide Episode Notification Record (DPR-ENF-114) [PENR] for reportable investigations. Updates include:

• Moving “death” from the Type of Episode to Basis.
• Replaced “in-patient status – less than 24 hours” with “in-patient status.”
• Removed “in-patient status – more than 24 hours.”
• Removed reference to Supplemental Form (DPR-ENF-116) since the form was discontinued.

The updated PENR will be available on DPR’s website. Please replace the previous versions of this form.
